- PART 1: Before You Inject the Hype—How Medical Research Actually Works
A mouse study is not a human trial. “Promising” is not the same as “proven.” And unknown risk is not the same as low risk. “Doc, what do you think about peptides?” I get asked this question almost every day. BPC-157. TB-500. CJC-1295. Ipamorelin. MOTS-c. New names seem to appear almost as quickly as new social media posts promoting them. Someone hears about a peptide on a podcast. An influencer says it transformed their recovery. A clinic promises better sleep, more muscle, faster healing, or even longer life. And suddenly, an experimental compound starts looking like established medicine. But before we talk about individual peptides, we need to understand one thing: How do we actually know whether a treatment works—and whether it is safe? I Am Not Anti-Peptide Quite the opposite. Peptides have already transformed medicine. Insulin is a peptide hormone. Modern incretin medications have revolutionized the treatment of obesity and diabetes. Other peptide-based drugs are used throughout endocrinology and many other areas of medicine. And some experimental peptides being discussed today may eventually become important therapies. But there is a huge difference between a medication studied in thousands of humans and a compound that produced an interesting result in a mouse. They may both be peptides. They do not have the same level of evidence. How Does an Idea Become Medicine? Medical research usually moves through several stages: Laboratory research → Animal research → Human trials → Regulatory review → Ongoing safety monitoring Each step answers different questions. Laboratory Research Researchers may first study a compound in cells or tissues. This can help show how something might work. But a cell in a laboratory dish is not a person. A molecule entering the human body may affect the immune system, metabolism, hormones, organs, and biological pathways that were never part of the original experiment. So when someone says: “Studies show this works.” My first question is: What kind of studies? Animal Research Animal studies are valuable. They can help researchers understand biological effects, dosing, metabolism, and possible toxicity. But animals and humans are different. Something that dramatically improves healing in a mouse may not work in a human. Something that appears safe during a short animal experiment may create problems when people use it for months or years. So when I see exciting animal research, my response is not: “It doesn't work.” My response is: “Great. Now let's study humans.” That is how science is supposed to work. Then Come Human Trials Even human research exists on different levels. Phase 1 Early studies primarily examine safety, dosing, and how the body processes a drug. Phase 2 Researchers begin asking whether it appears to work. Phase 3 Larger studies provide much stronger information about benefits and risks. This distinction matters. A compound being “studied in humans” does not mean it has been proven effective. Twenty people using something for a few weeks cannot tell us what may happen when thousands of people use it for years. Study size matters. Study duration matters. What researchers actually measured matters. A Better Number Does Not Always Mean a Healthier Person This is another place where hype can get ahead of science. A peptide may change a hormone level, inflammatory marker, growth factor, or body-composition measurement. Interesting? Absolutely. But the more important questions are: Did people actually feel better? Did they heal faster? Did they function better? Did they avoid disease? Did they live longer? And what risks came with the benefit? A biological effect is not automatically a meaningful health outcome. The Most Dangerous Phrase: “There Is No Evidence It Is Dangerous” Be careful. That statement can mean: We studied this extensively and have not found a major safety problem. Or it can mean: Almost nobody has studied this properly, so we have no idea what the risks are. Those are completely different situations. UNKNOWN RISK ≠ LOW RISK Not knowing that something is dangerous is not the same as knowing that it is safe. And What Is Actually in the Vial? This question is often overlooked. Even when a particular molecule is scientifically interesting, the product being sold still matters. Is it actually what the label says? Is the dose accurate? Is it sterile? Is it contaminated? Has it been stored correctly? A scientific study using a carefully manufactured product does not automatically validate every vial carrying the same peptide name. The molecule matters. But so do purity, sterility, dose, and manufacturing quality. Then Came Social Media Social media has helped patients become more informed and engaged. But it has also created a major problem: Social media moves much faster than science. A good clinical trial may take years. A viral video takes seconds. And the algorithm does not ask whether a treatment has been properly studied. It asks whether you clicked. That is why repetition can begin to look like proof. But: Popularity is not a phase of clinical research. Testimonials are not clinical trials. A million views do not equal one well-designed human study. My Position I am excited about the future of peptide medicine. Some of today's experimental compounds may become tomorrow's important therapies. Others will fail. Some may reveal risks we do not yet understand. That is normal. That is science. What concerns me is when we skip the process—when animal data becomes a treatment claim, a theoretical mechanism becomes a guarantee, and patients effectively become the experiment. Innovation is important. But we should never have to abandon scientific standards to pursue it. Before Trying Any New Peptide, Ask Five Questions Has it actually been studied in humans? How many humans? What benefit was actually proven? How long were people followed? Where did the product actually come from? Those five questions can eliminate a remarkable amount of medical misinformation. The Bottom Line I believe peptide medicine has an exciting future. But the future of medicine should be built on evidence—not algorithms. We should be open to innovation while remaining honest about what we know, what we do not know, and what may still carry unknown risk. Sometimes the most responsible answer a physician can give is: “We don't know yet.” That may not go viral. But it may keep patients safe. Next: Part 2—What Exactly Is a Peptide? “Doc, what do you think about peptides?” I get asked this question almost every day. BPC-157. TB-500. CJC-1295. Ipamorelin. MOTS-c. Semax. Selank. GLP-1s. The list keeps growing—and social media is moving much faster than the science. Some peptides have transformed modern medicine. Others are being injected into people with surprisingly little meaningful human data. Those are not the same thing. Over the next several weeks, I’m launching a 5-part series on peptides on BHInstitute.com that will take a hard, scientific look at what we actually know. A million views do not equal one well-designed human clinical trial. And “promising” should never be confused with “proven.” As physicians, our responsibility is not to chase every trend. It is to understand the science, evaluate the evidence, protect our patients, and still remain open to where medicine is going next. The future of peptide medicine may be incredibly exciting. But we should never have to sacrifice patient safety just to get there first. Artificial intelligence is quickly becoming one of the most talked-about topics in health care. Patients are hearing that AI can review scans, interpret lab results, predict disease risk, summarize medical records, and even suggest possible diagnoses. - Cancer Screening Is Changing: What New ASCO 2026 Data on Multi-Cancer Detection and GLP Medications Means for Patients
Cancer care is undergoing a major shift. For decades, medicine has focused on finding cancer after symptoms appear or after a cancer has already advanced. Today, two major areas of research are pushing us toward a more proactive model: detecting cancer earlier and reducing the metabolic conditions that may increase cancer risk or progression. At the 2026 American Society of Clinical Oncology Annual Meeting, two topics stood out: new data on the GRAIL Galleri® multi-cancer early detection blood test and emerging research on GLP-1 medications, including semaglutide and tirzepatide, and their possible relationship to cancer outcomes. At Beverly Hills Institute, we believe the future of preventive medicine is not simply treating disease after it appears. It is identifying risk earlier, acting sooner, and helping patients live longer, healthier lives. The GRAIL Galleri Test: A Blood Test Designed to Look for More Than 50 Cancers The Galleri test is a blood test designed to detect cancer signals from DNA shed into the bloodstream by cancer cells. It is not a genetic risk test. It does not tell you whether you may develop cancer in the future. Instead, it looks for signals that may suggest cancer is already present and may help predict where in the body that cancer signal is coming from. This matters because many dangerous cancers do not currently have routine screening tests. We routinely screen for breast cancer, colon cancer, cervical cancer, prostate cancer in selected patients, and lung cancer in high-risk smokers. But many cancers, including pancreatic, ovarian, liver, esophageal, and other aggressive cancers, are often found late because there has not been a practical screening tool for the general population. Galleri is intended to be used in addition to, not instead of, standard recommended cancer screenings such as mammography, colonoscopy, PSA testing when appropriate, cervical cancer screening, and lung cancer screening for eligible patients. What Was Presented at ASCO 2026? At ASCO 2026, GRAIL presented encouraging new data from two major studies: the NHS-Galleri trial and the PATHFINDER 2 study. Together, these studies suggest that the Galleri multi-cancer early detection blood test may help find more cancers earlier, including cancers that do not currently have routine screening options. The NHS-Galleri trial included more than 142,000 adults ages 50 to 77 in England. While the study did not meet its primary endpoint of reducing the combined number of stage III and stage IV cancers, the results showed important benefits over repeated annual testing. Among 12 prespecified deadly cancers, Galleri reduced stage IV cancer diagnoses by 9% in the first screening round, 22% in the second round, and 26% in the third round, with an overall 14% reduction after three rounds. The study also showed a 16% increase in stage I and II cancer diagnoses, a four-fold increase in screen-detected cancers, and a 25% reduction in cancers diagnosed after emergency presentation. These findings suggest Galleri may help shift cancer detection earlier, when patients may have more treatment options and better outcomes. The PATHFINDER 2 study included 35,878 adults age 50 and older in the United States and Canada. When Galleri was added to standard recommended screening, cancer detection by screening increased 6.5-fold. More than half of the new cancers detected by Galleri, 53.0%, were stage I or II, and 70.9% were detected at stages I to III. Importantly, 71.3% of the new cancers detected had no USPSTF A or B recommended screening option. Galleri also showed strong test performance in PATHFINDER 2, with a positive predictive value of 60.3%, specificity of 99.6%, a false-positive rate below 0.4%, and cancer signal origin accuracy of 91.3%. The key message is that Galleri does not replace standard cancer screening, but the 2026 ASCO data supports its role as an additional tool that may help detect more cancers earlier, reduce stage IV diagnoses over time, and identify cancers that are often missed until they are advanced. What Does This Mean for Patients? The most important message is this: early detection matters. Cancer outcomes are often dramatically different when cancer is found before it spreads. A stage I or II cancer may be treatable with curative intent. A stage IV cancer is often much harder to treat and may require systemic therapy, complex treatment decisions, and long-term disease management. The Galleri test is not perfect. A negative result does not rule out cancer. A positive result requires appropriate follow-up testing, such as imaging or other diagnostic evaluation. False positives and false negatives can occur. The test has also not been cleared or approved by the FDA, although it is performed in a CLIA-certified, CAP-accredited laboratory. But the new ASCO data adds to a growing body of evidence that multi-cancer early detection may become an important part of a modern preventive medicine strategy. Beverly Hills Institute Recommendation At Beverly Hills Institute, we recommend the GRAIL Galleri multi-cancer early detection blood test annually for patients over age 50. We also recommend considering annual Galleri testing for patients under age 50 who have a personal history of cancer or a significant family history of cancer. This recommendation does not replace standard screening. Patients should still complete appropriate routine screening, including colonoscopy or stool-based colon cancer screening, mammography, cervical cancer screening, prostate cancer screening when appropriate, dermatologic skin checks, and lung cancer screening for eligible patients. Our approach is additive: use the best standard tools available, and when appropriate, add newer technologies that may help detect cancers that routine screening may miss. GLP-1 Medications and Cancer: Another Important ASCO 2026 Topic Another major topic at ASCO 2026 was the relationship between GLP-1 receptor agonist medications and cancer progression. These medications include drugs such as semaglutide and tirzepatide, which are widely used for type 2 diabetes, obesity, and medical weight management. A real-world analysis presented at ASCO evaluated 12,112 patients with stage I to III cancer and compared patients who received GLP-1 medications with patients who received DPP-4 inhibitors. The cancers studied included breast cancer, prostate cancer, non-small cell lung cancer, colorectal cancer, liver cancer, kidney cancer, and pancreatic cancer. The study found that GLP-1 use was associated with reduced progression to stage IV disease in several obesity-related cancers. Statistically significant reductions were seen in non-small cell lung cancer, breast cancer, colorectal cancer, and hepatocellular carcinoma. For example, progression to stage IV disease occurred in 10% of GLP-1 users versus 22% of DPP-4 inhibitor users in non-small cell lung cancer, 10% versus 20% in breast cancer, 13% versus 22% in colorectal cancer, and 19% versus 28% in hepatocellular carcinoma. The study also found that higher tumor GLP-1 receptor expression was associated with a 33% lower risk of death across the tumor types studied, and a 45% lower risk of death in breast cancer. This does not prove that GLP-1 medications prevent cancer progression. The study was observational, meaning it can show an association but cannot prove cause and effect. Researchers appropriately noted that randomized controlled trials are needed. Still, the findings are fascinating and consistent with a broader medical reality: obesity, insulin resistance, inflammation, and metabolic disease are deeply connected to cancer risk and overall health. The Bigger Picture: Prevention, Metabolism, and Early Detection The future of medicine will likely combine several strategies: Earlier cancer detection Better use of imaging and blood-based screening Personalized risk assessment Metabolic optimization Weight management Exercise and muscle preservation Reduction of inflammation and insulin resistance More aggressive prevention before disease becomes advanced At Beverly Hills Institute, this is exactly how we think about preventive care. Final Thoughts The ASCO 2026 data does not mean we have solved cancer screening. It does not mean one blood test can replace colonoscopy, mammography, PSA testing, cervical cancer screening, or other proven tools. But it does show that cancer screening is evolving. The GRAIL Galleri test may help identify cancers that do not currently have routine screening options. GLP-1 medications may have broader effects on metabolic and cancer-related outcomes than previously understood. Together, these developments reinforce a central idea: prevention and early detection are becoming more powerful. - Your Cholesterol Number May Not Tell the Whole Story
Why Advanced Heart Risk Testing May Be One of the Most Important Preventive Health Conversations This Year For decades, patients have been told to “check your cholesterol.” Most people know their LDL cholesterol, often called “bad cholesterol,” and HDL cholesterol, often called “good cholesterol.” But the newest cholesterol guidance from the American College of Cardiology and American Heart Association makes something very clear: traditional cholesterol testing is important, but it may not tell the whole story. The updated 2026 cholesterol guideline emphasizes a more personalized approach to cardiovascular prevention, including earlier treatment, clearer LDL cholesterol goals, and selective use of additional tests such as coronary artery calcium scoring, Lipoprotein(a), and Apolipoprotein B. These tools can help identify hidden cardiovascular risk before a heart attack or stroke occurs. At Beverly Hills Institute, this is exactly the kind of prevention we discuss during an Annual Wellness Visit: not just whether your cholesterol is “normal,” but whether your overall cardiovascular risk is truly being understood. Why “Normal Cholesterol” Can Be Misleading A standard lipid panel usually includes total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. These numbers are very useful, but they do not always fully capture a patient’s true risk. Two people can have the same LDL cholesterol level but very different risk profiles. One may have no plaque in the arteries, while the other may already have early coronary artery disease. One may have a strong family history of heart attack. Another may have diabetes, elevated inflammatory risk, high triglycerides, or a genetically elevated Lipoprotein(a). That is why modern preventive medicine is moving beyond simply asking, “Is your cholesterol high?” and instead asking, “What is your actual risk of developing plaque, heart attack, or stroke?” LDL Goals Are Back One of the most important updates in the 2026 guideline is the return of LDL cholesterol treatment goals. For prevention of a first heart attack or stroke, the guideline identifies LDL-C goals based on risk level. For many patients at borderline or intermediate risk, the LDL-C goal is less than 100 mg/dL. For high-risk patients, the goal is less than 70 mg/dL. For patients with known cardiovascular disease who are at very high risk, the goal may be less than 55 mg/dL. This matters because cardiovascular disease is not something that suddenly appears at age 65. Plaque builds over years and decades. The earlier we identify risk, the more opportunity we have to intervene with lifestyle, nutrition, exercise, weight management, blood pressure control, diabetes prevention, and when appropriate, medication. Coronary Artery Calcium Scan: Seeing Plaque Before Symptoms A coronary artery calcium scan, often called a CAC scan, is a low-radiation CT scan that looks for calcified plaque in the coronary arteries. It does not require contrast dye, and it can help determine whether a patient already has early evidence of coronary artery disease. The new guideline supports selective use of CAC scoring in men age 40 and older and women age 45 and older who have borderline or intermediate 10-year cardiovascular risk when the result would help guide a decision about statin therapy. Any amount of coronary artery calcium supports a more serious discussion about LDL lowering, with lower LDL goals recommended as the calcium score increases. In practical terms, this test can be very helpful for patients who say: “My cholesterol is borderline, but I don’t want to take medication unless I really need it.” Or: “My cholesterol does not look that bad, but heart disease runs in my family.” A CAC scan can help move the conversation from guesswork to evidence. Lipoprotein(a): The Genetic Risk Many Patients Have Never Checked Lipoprotein(a), or Lp(a), is a cholesterol-related particle that is largely genetically determined. Unlike LDL cholesterol, it is not significantly changed by diet, exercise, or weight loss. That does not mean lifestyle is unimportant. It means that if Lp(a) is high, we need to know about it so we can manage the rest of the patient’s risk more aggressively. The 2026 guideline states that Lp(a) should be measured at least once in adulthood. A high Lp(a), defined as 125 nmol/L or greater or 50 mg/dL or greater, is associated with increased long-term risk of heart attack and stroke. Very high levels are associated with even greater risk. This is especially important for patients with: A family history of early heart diseaseA personal history of unexplained cardiovascular diseaseHigh cholesterol despite a healthy lifestyleAortic valve diseaseA heart attack or stroke at a younger age Many patients with elevated Lp(a) have no idea they carry this risk because it is not included in a routine cholesterol panel. Apolipoprotein B: Counting the Dangerous Particles Apolipoprotein B, or ApoB, is another advanced marker that can help refine cardiovascular risk. ApoB reflects the number of atherogenic particles in the blood, meaning the particles that can enter the artery wall and contribute to plaque formation. In some patients, LDL cholesterol can look acceptable while ApoB remains elevated. This is more common in people with insulin resistance, metabolic syndrome, diabetes, high triglycerides, obesity, or cardiovascular-kidney-metabolic risk. The guideline notes that ApoB may be useful to assess residual cardiovascular risk and guide treatment in certain patients, particularly those with type 2 diabetes, high triglycerides, cardiovascular-kidney-metabolic syndrome, or known cardiovascular disease. In other words, ApoB may help answer a more precise question: how many plaque-forming particles are actually circulating in the bloodstream? Prevention Is No Longer One-Size-Fits-All The most important message from the new cholesterol guidance is that prevention should be personalized. For some patients, a standard lipid panel and healthy lifestyle counseling may be enough. For others, especially those with family history, metabolic risk, diabetes, elevated triglycerides, obesity, inflammatory conditions, or unclear risk, additional testing can provide a much clearer picture. This does not mean every patient needs every test. It means the right patient should get the right test at the right time.
